Quality Assurance Review Training Program Registration

Please complete registration form, then e-mail registration form to the Registrar at:

You can complete the registration form on-line.  To e-mail the completed registration form, click on FILE and select SAVE AS from the Command Bar.  You can save a copy of this form on your desktop or somewhere you can easily find it.  You can then attach it to an e-mail and send it to me for processing.

Once your registration form has been received and processed, a copy will be sent to your Chief Internal Auditor.

Upon receipt of your registration, a confirmation will be sent to you via e-mail (when applicable) granting you access to the Quality Assurance Review On-Line Course.
